Purpose of Position/Summary:

To Providenursing care services to Indiana residents that follow infection controlpractices as guided by the CDC, CMS and state regulations. This position will provide essentialassistance during periods of increased patient volume or emergencies.

Essential Duties/Responsibilities:

• Provide onsite point of care services such asvaccinations and testing. Follow all agency and division policies andprocedures relating to safety, infection control, documentation, andeligibility. Document and reportconcerns as directed to agency leadership.

• Maintain proficiency in all related nursingskills. Complete all required trainings and in-services and maintain allrequired licensure and certifications.

• Maintain confidentiality of all patients.

• Complete required documentation related to allaspects of care provided.

• Document findings and submit reports withinrequired timeframes.

• Attend required staff and Division meetings.

• Other duties as assigned

Job Requirements:

• Must at least hold an LPN license; RN licensepreferred. Must be knowledgeable ininfection control practices, vaccinations, point of care testing, andphlebotomy. Will travel to various locations in Indiana and assist inthe operation of mobile clinics;

• Specialized knowledge of principles ofnursing practice and standards of care;

• Ability to work without onsite clinicalsupervision;

• Knowledge of nursing procedures and theability to work in a variety of settings;

• Ability to identify potential adverse healthand safety conditions;

• Ability to work as part of a team;

• Ability to utilize computer equipment andsoftware to enter, retrieve and manipulate data;

• Must hold and maintain a Valid Indiana’sDriver’s License;

• Ability and willingness to work before andafter standard work hours, including on weekends;

• Professional demeanor and ability to maintaincomposure under stressful situations;

• Commitment to the public health mission ofprotecting the health, safety, rights, privacy, and well-being of residents inlong term care facilities;

• Effectively communicate, both orally and inwriting

Difficulty of Work:

Must be able to travelthroughout an assigned region of Indiana. The ideal candidate would be willingto respond to surges in patient demand by being available for additional shiftsor on short notice.

Might have some overnightstays depending on location of outbreaks, and agency needs.

 

Responsibility:

The public health nurse playsa crucial role in community well-being. Responsibilities may include conductinghealth assessments, providing vaccinations, testing, offering health educationand providing treatment and care within diverse populations. They are requiredto provide care, and documentation consistent with all state and federalrules/regulations.

 

Personal Work Relationships:

Thisindividual may be expected to work alone or within team and will partner withagency personnel and related stakeholders working towards completion ofassigned functions. Exceptional customer service is provided to agency partners,facility partners, and/or the general public.

Physical Effort:

• This role requires the ability to operate amotor vehicle and drive short and long distances in all weather and lightconditions.

• Depending on the duties performed orenvironment setting, frequent walking, standing, climbing stairs, bending,stooping, pushing, pulling and/or similar movements may be involved.

• This role requires the ability tolift/transport items up to 20 lbs with or without the assistance of equipment.

• This role requires keen observational andhearing skills to stay alert for possible dangerous situations

 

Working Conditions:

Conditions may vary dependingon clinic location. Clinics may be held outdoors in a variety of weatherconditions. During outbreak response, all Personal Protective Equipment (PPE)must be worn according to agency policies and infection control practices mustbe followed at all times.


•Work will be accomplished as part of a larger mobile team.
•Individual will be assigned to a specific district mobile unit.
•Work will begin and end at a predetermined location.
•Personnel will ride to and from sites in the mobile unit unless otherwise authorized by direct supervisor.
•Agency will provide all supplies and equipment necessary to perform assigned tasks.
