Job Description:

Responsible for medication therapy management using in depth knowledge of patient care, pharmacy practice, pharmacotherpy, and technology. Coordinates with other heath care professionals to ensure optimal patient care through utilizing best practices. Provides education and mentorship to patients, pharmacists and other health care providers.

Principle Responsibilities

  • Coordinates improvement and quality opportunities.
    • Recognizes and addresses opportunities for improvement, using best practices information.
    • Uses quality improvement tools and techniques to make process improvements.
    • Participates in research studies, contributes to addition of practice body of knowledge, and presents findings at local, state and national meetings and through publication.
    • Works with leaders to optimize departmental financial performance.
    • Conducts audits on service quality and compliance when needed.
    • May lead development and implementation of new clinical practices, guidelines, policies.
  • Mentors and promotes the role of staff pharmacists.
    • Develops and maintains programs designed to improve the skills of pharmacists.
    • Participates in the education of physicians, nurses and other health care providers.
    • Participates in development of and training/orientation for new clinical services.
    • Assists in guiding, motivating, and holding people accountable for their practice.
    • Provides feedback and coaching on an ongoing basis.
    • Contributes feedback to managers to assist with the provision of performance evaluations.
  • Evaluates medication use.
    • Conducts medication use evaluations to identify opportunities to improve the use of medications.
    • Successfully collaborates with other health care professionals to accomplish improvements.
    • Participates in drug formulary reviews.
    • Submits articles to the Medical Staff Newsletter.
  • Coordinates resident and student education.
    • Maintains primary preceptorship of pharmacy residents.
    • Mentors staff pharmacist preceptorship of pharmacy residents in specialty rotations.
    • Directs student pharmacist programs in conjunction with staff pharmacists and department leaders.
  • Monitors patient and employee safety.
    • Provides patient education.
    • Identifies and addresses patient safety issues.
    • Participates in root cause analysis for medication error occurrences.
    • Participates in review of adverse drug events and identifies and executes plans to reduce the likelihood in the future.
  • May lead specialty practice areas.
    • Provides expert clinical consultation services in the areas of pharmacology, immunotherapy, immunology, transplantation, infections disease, oncology/investigational drug, pain management and palliative care, critical care, cardiovascular care and ambulatory care.
    • Provides leadership in the collection, analysis and communication of patient outcomes data.
    • Serves as a resource to department and hospital staff through education programs and materials.

Required Qualifications

  • Bachelor's degree in Pharmacy or
  • Doctoral/Ph.D. level advanced degree in Doctor of Pharmacy (PharmD)
  • Postgraduate year 2 residency experience or similar experience


Licenses/Certifications

  • Licensed Pharmacist - MN Board of Pharmacy required if working in the state of MN upon hire
  • Licensed Pharmacist - WI Board of Pharmacy required if working in the state of WI upon hire
