Home Health & Hospice Care is one of the oldest Visiting Nurse Agencies in the country tracing our roots to Nashua NH!  A community-based agency supporting 25 communities in Southern NH.  Specializing in homecare, hospice care and operates a 16 bed Community Hospice House.  Our Quality Clinical Specialist is key to helping us remain diligent in our quality department assisting with audits, OASIS, ICD-10 coding in accordance with CMS, federal and state regulations.  HHHC affiliated with Solution Health 2 yrs. ago!  We are now on EPIC and offer a generous benefits package, state of the art equipment, pleasant office environment and a great team!  Position is office based during orientation (90 days) once fully trained will transition to hybrid schedule minimum 2 days in office.

Responsibilities:



  1. Audits admission, resumption of care, recertification and discharge documentation (OASIS, ICD-10 coding, physician’s orders, plan of care) in accordance with CMS, federal, and state regulations.

  2. Reports accurately and concisely any inaccuracies in documentation to Homecare Quality Manager.

  3. Attends staff meetings and educates clinical staff on coding and documentation needs.

  4. Collaborates with different departments to ensure appropriate coding for OASIS and ICD-10 diagnoses.

  5. Completes compliance and quality audits timely and as instructed.

  6. Prepare documentation for Insurance inquires and denials, as needed.

  7. Verify Face to Face documentation.

  8. Precept new Quality team staff as assigned.

  9. Demonstrates flexibility by accepting other duties as assigned for which qualified.


  Qualification


Associates Degree in Nursing or Degree in Physical, Occupational or Speech Therapy


Professional license in good standing: Registered Nurse with NH Board of Nursing; Physical Therapist, Occupational Therapist or Speech Therapist with NH Board of Allied Health


Minimum of one year experience in home health care


Homecare regulation knowledge


 Preferred Qualifications



  • Bachelor’s Degree in Nursing or corresponding degree in Physical, Occupational, or Speech Therapy.

  • 2+ years experience in home health care.

  • OASIS certification.

  • ICD-10 diagnosis coding certification.

  • Expert in auditing clinical documentation.

  • Experience with EPIC.


Required



  • Registered / licensed clinician in NH as RN, PT, OT or SLP.

  • Satisfactory criminal record.

  • Pre-employment physical plus annual flu vaccine unless provided with religious or medical exemption.
