The Division of Mental Health is seeking to hire a position to perform professional social work duties as a Transition-Age Youth Clinical Specialist for children, adolescents, and families who need assistance through the Children’s Behavioral Health Care Portal, specifically targeting adolescents aged 18-22 who are navigating the transition between child/adolescent and adult service delivery systems. This position provides technical assistance for specialized mental health service programs; conducts assessments of individuals and families, provides clinical consultation and coordinates linkage to statewide programs; and collaborates with inter-agency team members and completes reports and documentation.
Of the two (2) years of professional social work experience, requires two (2) years of experience working with children and/or adolescents.

Work Hours: 8:30am - 5:00pm, Monday-Friday, 1-hour unpaid lunch
