Clinical Transplant Pharmacist - Solid Organ Specialist

Position Overview

As our Transplant Pharmacist, you'll play a crucial role in optimizing patient outcomes through expert medication management and clinical decision-making throughout the transplant journey.

Key Responsibilities

  • Provide specialized pharmaceutical care across the transplant continuum, from evaluation to long-term follow-up
  • Optimize immunosuppressive therapy and manage complex medication regimens
  • Participate in multidisciplinary transplant selection committees
  • Conduct pre-transplant medication counseling and post-transplant education
  • Monitor and manage medication therapies for transplant recipients
  • Develop and implement treatment protocols for allograft rejection
  • Precept pharmacy students and residents in transplant medicine
  • Lead clinical research and quality improvement initiatives

Required Qualifications

  • PharmD from an ACPE-accredited institution
  • Virginia pharmacist license or eligibility for licensure within 90 days
  • Completion of ASHP-accredited PGY1 Pharmacy Residency
  • Strong clinical decision-making and analytical skills
  • Excellence in interprofessional communication

Preferred Qualifications

  • PGY2 Solid Organ Transplant Residency
  • Prior transplant pharmacy experience
  • Research experience in transplant medicine
