Essential Functions:

  • Demonstrate the values and guiding principles of OhioGuidestone every day, performing with the highest standards of excellence, integrity, and ethics.
  • Provide clinical oversight for the OhioRISE Wraparound Program, ensuring fidelity to the Wraparound model.
  • Support Care Coordination Supervisors and Care Coordinators working with youth with high acuity needs.
  • Maintain clinical oversight on high acuity cases by reviewing documentation, attending Child and Family Team (CFT) meetings, and participating in regular clinical case reviews.
  • Provide initial support to Care Coordinators and Supervisors in evaluating care needs and crisis response when client needs rapidly escalate.
  • Establish and facilitate regular clinical reviews of high acuity needs cases to ensure alignment with best clinical practices and OhioRISE Wraparound fidelity.
  • Support the development of Care Plans, Crisis Safety Plans, and transition/discharge strategies specifically for high acuity youth.
  • Review and sign Safety Plans to ensure alignment with clinical standards and risk management practices.
  • Participate in weekly leadership meetings as required, functioning as a member of the Leadership Team alongside Supervisors and Assistant Directors.
  • Participate in quality assurance and quality improvement activities as assigned.
  • Ensure community-based services are maximized and that all lower levels of care are considered before more restrictive placements.
  • Escalate system-level barriers to the CME Director and Aetna, including stakeholder engagement concerns or issues affecting wraparound fidelity.
  • Attend required trainings and meetings as designated by Aetna, the Center of Excellence (COE), and ODM.
  • Exhibit positivity, flexibility, and a willingness to take on new responsibilities as requested or required.
  • Demonstrate positive leadership, promote a team-based work environment, and foster a culture of belonging.
  • Present the Agency in the most positive light with all internal and external contacts.

 

Education & Experience:

  • Master’s Degree required
  • Independently Licensed Behavioral Health Clinician (LISW, LPCC, IMFT or equivalent); Supervisor Designation preferred
  • Must complete all OhioRISE and COE-required trainings for Moderate and Intensive Care Coordination and clinical supervision
  • Strong background in community-based care and youth with high behavioral health needs
